Our obligations:

We are committed to safeguarding the privacy of your medical information. We will notify any impacted individuals in case of a breach involving unsecured medical data, by applicable state and federal regulations. This Notice outlines our legal responsibilities and privacy practices regarding protecting your medical information.

How we may use and disclosure information about our patients:

Purpose for release of information:

Information requested - all medical records including, but not limited to: history and physical, consultation notes, inpatient, outpatient, and emergency room treatment, clinical charts, reports, order sheets, progress notes, nurse’s notes, clinic records, treatment plans, admission records, discharge summaries, laboratory records, requests for and reports of consultants, and correspondence.

Allied Clinic Policy:

We at Allied Clinic strive to create a friendly, comfortable environment where your and your family’s health is our primary concern.

Non-Compliance: It is the policy of this office that services provided to patients who do not comply with this office’s treatment plans/treatment recommendations will not be billed to their health care plans as such services are not consistent with “medically necessary care” and therefore, not covered by their health care plan (this includes, but is not limited to, patients who do not keep scheduled appointments and/or choose to seek care at their discretion and/or at their convenience). We have the right to dismiss a patient from this clinic/Provider and it will be the patient's responsibility to seek another provider.

Payments: We accept cash checks, debit, and credit cards as payment. There will be a $55.00 charge for all returned checks. Accounts must be current to continue to receive care from us. All past due balances are also due at the time of service or make arrangements.

Missed Appointments: $75.00 will be charged to you for missed appointments (unless a 24-hour notice is given). This also goes for no-shows. The missed appointment charge must be paid before or at your next visit. If you are going to be more than 10 minutes late, please give us a call so we are aware of it. Allied Clinic reserves the right to dismiss from the practice any patient who frequently misses scheduled appointments without prior notice.

Self-Pay Patients: If we are not contracted with your insurance or you do not have insurance we will require full payment at the time of service. The charge for consultation service is $150.00 this does not include any other blood draws and or strep, and other in-house testing.

Prescription Refill Requests: If you have not been seen in more than 3 months, an appointment and/or health check-up must be made before a complete refill of all scripts is given. Monitoring your health status is important to your well-being. Narcotics will not be refilled unless you are seen and assessed by the provider.

New Patients: Please arrive 15 minutes before your scheduled appointment to ensure the completion of your paperwork. The forms can be downloaded from our website and you can bring us or email the complete forms to info@alliedclinic.com.

Late for Appointments: If you arrive 15 minutes or more after your scheduled visit time, we reserve the right to reschedule your appointment.
